Summary of Benefits and Coverage
LifeWise Passport preferred provider organizations (PPO) plans offer a combination of up-front, first-dollar benefits and standard coverage for other services. A lower copay applies when the member visits their designated primary care doctor.
LifeWise Passport HSA plans offer valuable benefits for covered services and are qualified to work in combination with an employee owned, tax advantaged health savings account (HSA).
LifeWise Assure exclusive provider organization (EPO) plans offer a combination of up-front, first-dollar benefits and standard coverage for other services.
These plans were designed by the Oregon Department of Consumer and Business Services to be included in our plan offerings. They differ from the Passport plans in that they do not offer alternative care coverage.
All LifeWise small group plans are available for employees only.
LifeWise employer-sponsored dental plans meet the ACA requirement for providing pediatric dental essential health benefits. It's no secret good dental health affects your employees' overall health. LifeWise dental plans help both children and adults maintain healthy teeth.

HRAs are employer-owned and -funded arrangements that members can use to pay their cost-share for covered expenses. It is important to note that HRAs are not portable. If the employee leaves the company, any unused portion of the HRA (after all prior claims have been paid) reverts back to the employer.
FSAs are employer-sponsored, tax-advantaged healthcare spending accounts that members can use to pay for eligible expenses. Here are a few things to know about FSAs:

Forms are due by the 5th of the month
Completed enrollment materials for new groups must be received at LifeWise for HRA and HSA products for small groups beginning coverage on the first of the following month to ensure eligibility is active on the start date, and accounts are funded.
Completed renewal materials for renewing groups must be received at LifeWise for HRA and HSA products for small groups renewing contracts on the first of the following month to ensure eligibility is active on the start date, and accounts are funded.
LifeWise has selected USAble Life Inc., a company focused on supporting health plans with Life and Disability offerings, to assist groups to create an integrated benefits program to help reduce disability and healthcare costs, improve health and increase workforce productivity.
