    Traditional PPO plans

    These plans offer a combination of benefits for employees and a range of deductible, copay, coinsurance and price options. These plan designs offer rich first-dollar benefits and options for employers to choose the right level of cost-sharing to suit business needs.

    Passport PCP™

    Traditional PPO plans

    These plans offer the same combination of benefits as the Passport PPO plans with a wide range of deductible, copay, coinsurance and price options. But these plans offer a lower cost share for office visits when selecting then using a Primary Care Physician.

    LifeWise HSA™

    HSA-Qualified health plans

    High-deductible health plans, qualified to work in combination with a tax-advantaged Health Savings Account (HSA). These plans offer benefits for a wide range of services and are qualified to work in conjunction with an employee-owned HSA.

    LifeWise Universal™

    Traditional indemnity plans

    These plans offer maximum flexibility in provider choice and a range of cost-share options from which to choose.

    Optional benefits

    In addition to our health plans, you can enhance your employee's health benefits by selecting these additional buy-up options:

    Outpatient prescription drug plans

    Affordable prescription drugs are important to your employees' health and healthcare costs. Each plan includes coverage of generic prescriptions or you may have the option to purchase an alternative four-level plan. See the benefit guide for more information.

    Dental and orthodontia plans

    LifeWise dental plans feature a range of annual deductibles, maximum annual benefits, and coinsurance payments. For groups of five or more employees, available options include traditional, PPO, and self-funded plans.

    Enhanced benefits options for periodontal and endodontic services are available for groups of five or more employees. Orthodontia benefits are available to groups of 26 or more employees
